Boss SY-200
The SY-200 synth pedal over MIDI, with direct and effect level, the three assignable parameter CCs, the three CTL CCs, expression and the effect select. The parameters each do something different depending on the type selected, which is why they are named by CC rather than by function.
Tested with the device Somebody has run this into the gear it was made for.
